TEACHER/PARENT DISCUSSION CHECKLIST For one activity, not permission to deploy a service. 1. What learning task should the learner be able to explain or complete? 2. What specific access, attention, answer-quality, information or workload concern occurred? 3. What is observed, reported, inferred or still unknown? 4. Which equivalent accessible route preserves instructions, feedback and help? 5. What information would leave the approved environment, and who decides whether that is permitted? 6. What failure would pause the activity? Who owns the next step and review date? FICTIONAL STAFF EXAMPLE Task: learners explain fraction addition. Existing hint sheet: assumed 30 staff-minutes. Drafting route: 10 setup + 8 generation + 18 review/correction = 36 minutes. A scripted hint gives 3/4 for 1/2+1/4 but says add both numerators and denominators, which would give 2/6. Reject the explanation; no student use or measured saving is established. Keep the existing accessible route and seek the authorised school lead's decision. FICTIONAL PARENT EXAMPLE A learner cannot reliably open a homework service. Ask: What equivalent format can we use, how will feedback reach us, and who should we contact if that also fails? Record the agreed route and date. Do not publish student information in this worksheet. BLANK RISK RECORD Activity / learners and setting / date: Intended learning task and existing approach: Specific risk and who could be affected: Source or observation / date / what it cannot establish: Baseline: learning evidence, access and separate effort/cost measures: Proposed change and an equivalent accessible alternative: Data needed / destination / unresolved approval questions: Potential mitigation / evidence it has actually been implemented: Observable harm or failure signal / condition for pausing: Responsible person and authorised decision-maker: Decision / unresolved issues / next review date: What can be shared safely, with whose permission:
